1. Introduction
Precision Audiology Australia ("we," "our," "us") is committed to protecting the privacy and
confidentiality of personal and health information. We adhere to the Privacy Act 1988 (Cth),
including the Australian Privacy Principles (APPs), and comply with the requirements of the
Hearing Services Program (HSP) as administered by the Australian Government Department
of Health and Aged Care.
2. Collection of Personal Information
We collect personal and health information necessary to provide audiology services,
including but not limited to:
− Full name, date of birth, contact details
− Residential address
− Emergency contact details
− Pension card or concession details
− HSP eligibility information
− Clinical notes
− Hearing aid serial numbers
− Device fitting and maintenance records
− Outcome measures
− Payment and claiming information (where applicable)
− Medicare and Department of Veterans' Affairs numbers
− Audiological assessments and medical referrals
− Hearing device usage and maintenance records
Information is collected directly from clients or their authorized representatives, and, where
applicable, from other healthcare providers with the client's consent.
3. Use and Disclosure of Personal Information
Personal information is used to:
- Provide and manage hearing services
- Fulfill obligations under the HSP
- Communicate with clients regarding their care
- Comply with legal and regulatory requirements
We disclose personal information to:
- The Department of Health and Aged Care for HSP administration
- Other healthcare professionals involved in the client's care
- Third-party service providers who assist in our operations, under strict confidentiality
agreements
Some electronic service providers used by Precision Audiology Australia may store or
process information on servers located outside Australia. Where this occurs, we take
reasonable steps to ensure personal information is protected in accordance with Australian
privacy laws.
CounselEAR stores client information on an Australian Server.
4. Data Breach
If Precision Audiology Australia becomes aware of a suspected or actual data breach, we will
investigate promptly and take appropriate steps to contain the breach. Where required
under the Notifiable Data Breaches Scheme, affected individuals, the Office of the Australian
Information Commissioner (OAIC), and relevant government agencies will be notified.:
5. Data Security and Storage
We implement robust security measures to protect personal information from unauthorized
access, modification, or disclosure. These measures include:
- Secure electronic record systems with access controls
- Physical security for paper records
- Regular staff training on privacy obligations
Clinical records are retained for the minimum periods required under applicable
Commonwealth and State legislation, professional standards, and Hearing Services Program
requirements before being securely destroyed.
6. Access and Correction
Clients have the right to access their personal information and request corrections if they
believe the information is inaccurate, incomplete, or outdated. Requests can be made by
contacting our Privacy Officer (details below). We will respond to such requests in a timely
manner, as required by the APPs.
7. Consent and Client Rights
We obtain informed consent before collecting, using, or disclosing personal information,
except where permitted or required by law. Clients have the right to:
- Clients may withdraw consent for future use or disclosure of their information at any time,
subject to legal, clinical, contractual and Hearing Services Program obligations
- Request access to their personal information
- Lodge a complaint if they believe their privacy has been breached
